Thank you for taking the time to fill out the 2012-13 FHSAA sports participation survey. Please follow all instructions below carefully before filling out the form. Each member school must report the total number of participants on each interscholastic team throughout the school year by grade level and gender. An interscholastic sport is a sport funded in whole or in part by your school under the direct supervision of the principal and athletic director, and in which your school fielded a team and played a representative schedule against teams from other schools. You must include all students that participated on the following levels, but not limited to: varsity, junior varsity, freshman and middle school. Do not report students that participated in intramural or club sports programs. To get started you must select your school, name of person filling out the form, submitter’s e-mail address and position within the school. Then check each sport that your school sponsors on any level. That includes, but is not limited to varsity, junior varsity, freshman and middle school. (If you have three different boys basketball teams, you only need to check the “Basketball – Boys” box once) After selecting all sports at the school click on “Next Page” at the bottom. The e-mail address provided on this form will get a confirmation e-mail that the submission was received. Your school’s name will not disappear from the dropdown menu on the instruction page. The confirmation e-mail will serve as notice that the submission was successful.
