Report miscellaneous situations: facilities issues, event cancellation, injuries, spectator misconduct, equipment/uniform compliances or any other incident not previously mentioned.
This form must be submitted within 24 hours of an incident to the FHSAA Associate Executive Director for Athletic Services. SERIOUS ACTS OF UNSPORTSMANLIKE CONDUCT SUCH AS BENCH EMPTYING AND CONTESTS THAT DO NOT GO TO THEIR NORMAL CONCLUSIONS MUST BE REPORTED IMMEDIATELY BY CALLING (352) 372-9551, EXT. 300 AND SUBMITTING THIS REPORT.
