Winter 2008 Sports Participation Survey

Below are the 6 member schools that have yet to submit the Winter 2008 Sports Participation Survey to the FHSAA Office. The original deadline was Mar. 28, 2008. The second deadline for submission of this report was Friday, Apr. 18, 2008. Schools that did not file by the second deadline were assessed a $100 late fee. There was a third deadline of Thursday, June 19, 2008. If surveys were not turned in by that date, additional fines were levied. Please fax the completed form as soon as possible to (352) 373-1528.
