With the Sept. 18, 2009, deadline now passed, the following schools have been fined for not turning in the necessary paperwork. There was a second deadline set for Wednesday, Sept. 30, 2009. Any school on the list after Sept. 30 was fined again. The third deadline is now Friday, Oct. 9, 2009. If your school's name is on this list, please turn in the paperwork as soon as possible. If you are having fax problems, the documents can be scanned an e-mailed to Seth Polansky, Membership Services Specialist, at spolansky@fhsaa.org. If you have previously faxed these documents in and have a confirmation page to prove it, please send those along as well and your fine will be waived.

Please keep in mind that the phone and fax lines at the FHSAA Office were down Thursday, Aug. 13 to Monday, Aug. 17, and had sporadic problems through that week. If you tried to send forms at that time, it might not have gone through.

If your school's name is on this list, one or both of your forms are due. If a form was tuned in already but incorrect, the date it was returned back to the school is listed. If your school’s name is not on any list, all your forms have been turned in and processed.

The GA1 form is to order coaches ID cards. This is an important document for us as we need to reach coaches in emergencies. Therefore, for every coach listed on the sheet, there must be a cell number and an e-mail address listed. If the coach does not have a cell phone, then the cell phone number for anyone in the travel party can be used. If a coach is in charge of multiple teams, he/she must be listed every single time. Writing “same as above” or just using ditto marks(“”) is not allowed. If there isn't a coach hired for a particular sport at this time, put “TBA.”

The FHSAA representative must sign in the top section of page 1. Both pages must be sent even if your school does not have any sports listed on the second page. Do not alter the document, use last year's form or add names we are not asking for. Any errors with the form and it will be mailed back to the school.

The GA3 form is the certification of compliance with FHSAA policy on athletic recruiting. This needs to be signed by every administrator, coach of a fall sport and booster club president for the school.

Every administrator and coach listed must have their signature next to their name. If a coach is in charge of multiple teams or an administrator serves two functions, he/she must be listed and sign every single time. Writing “same as above” or just using ditto marks(“”) is not allowed. If the athletic director or principal is also the FHSAA representative, then his/her name and signature must be on both lines. No rubber stamp signatures allowed. If there isn't a coach hired for a particular sport at this time, put “TBA” and resubmit the form with the proper signature(s).
