Below are lists of schools that have yet to turn in continuing membership information to the FHSAA for the 2008-09 school year or need to turn in additional items. The column on the left indicates schools that have not turned in a membership application. The column on the right indicates schools that had missing or incomplete information from their membership applications. It includes what documents need to be returned after the school name. All applications and missing information must be turned into the FHSAA as soon as possible or risk suspension from membership for the upcoming school year.

All documents needed can be found in the yellow box before the list and must be typed to be accepted, even ones that need a signature to be complete. All missing items can be found in the MB02 Continuing Membership packet except for the AED Compliance Form, which is just below it. Everything can be faxed to (352) 373-1528.
