EL4 and EL7 forms revised; EL12 form created (students at nonmember schools participating at home-zoned public school)

Due to recent changes in policy and state legislation, the FHSAA has revised the EL4 and EL7 forms, while creating the EL12 form. The new EL12 is in reference to state law allowing students at private nonmember schools that have certain parameters to participate in interscholastic athletics at their home-zoned public school. The forms are below.
