Important Notice Regarding Forms
Forms are provided either as Portable Document Format (PDF) files, Microsoft Word documents, or both, all of which can be downloaded into, completed on, and printed out of your computer.
When working with PDF files, please be aware of the following:
1. Download the file onto your computer hard drive. Do not attempt to complete the form on your web browser. To SAVE the publication, first allow the document to load completely, then use your browser's built-in controls ("File" menu, "Save As . . .") to save the file to your computer's hard drive.
2. Save often as you complete the form. If you plan to use the form more than once, use the "Save As" function to rename the file so that the blank original is retained on your hard drive.
3. Do not e-mail the completed PDF form. E-mailed PDF files do not retain the information entered into them and are received blank by the addressee. MS Word documents, on the other hand, may be completed and e-mailed as attachments.
IMPORTANT: If you download a form and the page is blank and/or generates errors, you probably need to upgrade your version of Adobe Acrobat ® Reader. To get a new version of Adobe Acrobat ® Reader, click here.
