Event Approval Form
AT2 : Application for Approval of Athletic Event (baseball, basketball, soccer, softball, girls volleyball, wrestling) : PDF

This form requires that for invitationals the number of participating teams be confirmed before the 30-day filing. The Athletic Directors Advisory Committee members and the Board of Directors feel that it is a reasonable expectation that the number of participating teams can be determined in this time frame.

The school will be invoiced after the form has been approved by the FHSAA Office. Please pay from invoice only.

Sanctioning fees are non-refundable!
As stated on the approval form, the fee for sanctioning an event is non-refundable.

A request for a refund may be made for extenuating circumstances. A refund may also be made for errors in payment of the amount due. A reduction in the number of teams from what was originally anticipated or invited does not constitute an extenuating circumstance for the purpose of a refund. If you have a change prior to the 30-day approval form filing deadline and it reduces the fee, you may file an amended approval form and receive a refund. Please clearly mark the approval form “AMENDED”.

Stepped late fees for event approval requests
Effective with the 2005-06 school year, event approval requests received less than 30 days before the event will be charged the sanctioning fee plus a $50 administrative late fee. Event approval requests received less than 10 days before the event will be charged the sanctioning fee plus a $100 administrative late fee. Event approval requests received after the event will be charged the sanctioning fee plus a $200 administrative late fee.

Important Notes
• The event approval form is due 30 days in advance of the event
• The only payment to the FHSAA is that for the specific type of event (e.g., classic, jamboree or invitational) in the sports listed above.
• Subsequently, there is no requirement to file a financial report after the event.
