2008-09 membership forms now available
Applications must be received in FHSAA office by Wednesday, April 30
Applications for membership in the FHSAA for the 2008-09 school year were mailed out of the FHSAA office on Tuesday, March 25, to all continuing members, prospective new members and schools interested in received approved nonmember status. These forms must be typewritten and are now available as PDFs with fields that can be downloaded to your computer hard drive, typed and printed out. The links to the various forms are provided below. They also are available on the Membership Services page. Forms must be completed in their entirety. Incomplete forms, as well as forms without the necessary signatures, will be returned. Continuing members must ensure that all financial obligations to the Association have been satisfied. Prospective new members must submit all necessary documentation (e.g., board minutes approving membership, charter, certificates of insurance, etc.) with their application forms for the application to be considered complete.
