Physical evaluation, consent forms revised
The Preparticipation Physical Evaluation Form (EL2) and Consent and Release from Liability Certificate (EL3) have been revised. The physical form has been revised to reflect that the physical evaluation may be administered and signed by a licensed physician assistant. The consent form has been revised to include competitive cheerleading as one of the sports that is listed for parents to circle. We do not anticipate any other revisions to either of these forms for the 2008-09 school year.
